Thank you for allowing your child(ren) to participate in Vacation Bible School at The Vine Church! We are honored to care for and serve your family during this special week.
By checking the box below below, I give permission for my child(ren) to participate in VBS activities hosted by The Vine Church. I understand that while every reasonable effort will be made to provide a safe environment, accidents and unforeseen injuries can occur.



On behalf of myself and my child(ren), I release and hold harmless The Vine Church, along with its pastors, staff, volunteers, and representatives, from any claims, liabilities, damages, or expenses related to injuries, illness, or incidents arising from participation in VBS activities.
I understand that church volunteers and staff will supervise and care for my child(ren) during VBS programming and activities.
